How do I create a Microsoft Form for club attendance?

Log in to Office
Navigate to office.com and log in using your NRCA Credentials.

Click the 'Waffle' in the top left-hand corner

Choose 'Forms'
STEP
Choose 'New Form'
STEP
Click on 'Untitled Form' and give your form a name
STEP
Add in any of the options listed for information you would like to collect
We suggest using 'Choice' for the club dates and repopulate this year's club dates so you do not have to create a new form. If you would like for the students to upload a photo then choose 'Upload File'.

On the day of club, on a device in the meeting room pull up the form and click 'Present' to display the QR code
STEP
Students Scan the QR code; they MUST be logged in to their NRCA account to respond
Next month you will bring the QR code back up and they will choose the next date and then upload that month's selfie.
